What's Needed:
To complete the Prospect Profile, you will need:
- Agency name, address and contact numbers
- Agency FEIN number
- Current E&O Policy & License information
- Written Premium Information for your agency
- Principal, marketing, accounting and producer name, email and contact numbers.
Next Steps:
- Once your Prospect Profile is received (electronically, via fax, email or mail), within 2 business days, you will receive an email confirmation that your profile has been processed. After you receive the confirmation email, you can submit applications for quote, and, you should Create a User ID on our website to access comprehensive submission requirements, applications and detailed underwriting and program information.

- Your agency may recieve a call from a BISYS Specialty Programs representative to determine how our products & services can meet your client's needs.
Please remember that you will need to send:
- A copy of your producer license
- A copy of your E&O Policy declarations page
- A completed W9 form within 90 days of binding your first account or your registration will be suspended.
If signing up for BrokerWeb, please also include:
- A signed copy of the BrokerWeb Agent Agreement
- A signed copy of the Producer Online Agency Indemnification and Hold Harmless Agreement.
